What they are

Hyaluronic acid and how it works.

Dermal fillers used at Dr Winter’s clinic are based on hyaluronic acid, a substance that occurs naturally in the body and is responsible in part for skin hydration and volume. When injected, it temporarily adds volume and structure to specific areas. The effect is reversible.

Hyaluronic acid fillers are temporary. They are gradually broken down by the body over a period of months to years, depending on the area treated and the product used. In most facial areas, results last between six and eighteen months, though this varies considerably between patients.

Because hyaluronic acid fillers can be dissolved using an enzyme called hyaluronidase, they are considered reversible, an important consideration when choosing treatment that affects your appearance. Dr Winter will discuss this during consultation.

Anatomy-led approach

Why the face is more complex than it looks.

Facial anatomy is three-dimensional, variable between individuals, and changes with age in ways that are not always obvious from the surface. Volume loss occurs in layers, fat compartments, muscle, and bone, and these changes differ from person to person. A treatment approach that works well for one patient’s anatomy may be wrong for another’s.

Over-treatment with dermal filler, or filler placed in the wrong anatomical layer, produces results that look unnatural, a pillow-like or overdone appearance that was not the intention. Dr Winter’s background in clinical dermatology means that the assessment focuses on what is actually changing beneath the surface, not just what the patient dislikes about their appearance from the outside.

The consultation also considers when filler is not the appropriate solution. In some cases a skin quality treatment, collagen stimulation approach, or no treatment at this stage is the better recommendation.

Cheeks and mid-face

Volume reduction in the mid-face is common from the mid-thirties. Conservative filler placement here can restore facial balance without an artificial result.

Nasolabial folds

The folds from the nose to the mouth deepen as the face loses volume. Filler here is most effective when the underlying volume loss is addressed, not just the fold itself.

Lips

Lip filler discussion focuses on proportion and natural support rather than size. A conservative approach suited to the patient’s existing anatomy.

Jawline and chin

Definition in the lower face can diminish with age. Filler here is used selectively and only where anatomy supports a good outcome.

Every patient considering dermal filler treatment begins with a consultation appointment. During this, Dr Winter reviews your facial anatomy, the concern you want to address, your medical history, and what a realistic treatment outcome would look like for your specific face.

The consultation also covers the treatment process itself: where product would be placed, how much would be used, what the recovery period looks like, and what the realistic duration of results is for your anatomy and lifestyle.

Not every patient who attends a filler consultation leaves with a treatment plan. Some are better served by a different approach, some need more time to consider, and some are not appropriate candidates for treatment. The consultation exists to make that determination honestly.

After treatment

Results, recovery, and what to expect.

Following dermal filler treatment, some swelling and tenderness are normal and expected in the days immediately after injection. In some areas, bruising can occur. Dr Winter will explain what to anticipate in your specific case before treatment takes place.

Results improve as initial swelling settles over one to two weeks. The final result is typically visible at around the two-week mark. If there are concerns about the result at this stage, a review appointment can be arranged.

Dermal filler treatment, like all injectable procedures, should be approached with realistic expectations. The result is a subtle improvement in facial balance, not a transformation. If you are expecting a dramatic change, that expectation will be discussed and addressed during consultation before treatment is offered.

What are dermal fillers?

The face contains complex vascular anatomy, and treatment without a thorough understanding of those structures carries real risk. Dr Winter is a GMC-registered consultant dermatologist and BCAM full member with over 30 years of clinical experience. Every patient undergoes a medical assessment before treatment is offered, and not every patient is an appropriate candidate.

What areas can be treated with dermal filler?

A range of areas can be discussed at consultation, including the cheeks and mid-face, nasolabial folds, lips, jawline, chin, and under-eye area in appropriate cases. Suitability for each area is assessed individually. Not all patients are suitable candidates for treatment in every area, and Dr Winter will advise honestly on which areas may benefit and where a different approach would be more appropriate.

Can dermal filler be reversed?

Yes. Hyaluronic acid dermal fillers can be dissolved using an enzyme called hyaluronidase if this is required, whether the result was not as expected, a complication occurred, or dissolution is needed for clinical reasons. This reversibility is one reason hyaluronic acid is the preferred filler type for facial treatment. Non-hyaluronic acid fillers, including permanent or semi-permanent products, cannot be dissolved in the same way. Dr Winter does not use these products.

How long do dermal fillers last?

Duration varies depending on the area treated, the product used, and the individual patient. In most facial areas, results last between six and eighteen months, though this differs considerably between patients. Areas with more movement, such as the lips, tend to break down filler more quickly than areas with less dynamic activity. Dr Winter discusses expected duration during consultation based on your anatomy, the area being treated, and your lifestyle.
